Free sports physicals will be given on July 11th and 12th.
There are 22 spots available each day.
You must print and sign the documents listed below ("July Physicals" and "July Physicals 2") and return to Mrs. Piller in Room T2 by Tuesday, July 9th at 4:00pm in order to reserve a spot.
First come, first served. Appointment times start at 8:00am and end at 2:45pm with an hour for lunch.
Again, there are only 22 spots available each day. Don't miss out!
You can clear yourself completely for sports for the entire school year by getting this physical done and completing all of the paperwork posted on the Athletics Home page.
